Middle School (Modified) Spring Sports Begin- Monday, March 24, 2025

Registration Opens- Thursday, February 20, 2025

Registration Closes- Thursday, March 20, 2025

Physical Examination

Students registering for athletics must have a current physical examination before the student is authorized to participate in practices/tryouts. To ensure clearance for participation, students/parents/guardians should complete the following items in this online registration program at least 5 days prior to the first day of practice/tryouts:

• Upload a current Physical Examination Form  (If your doctor does not provide a form you may use the “NY Student Physical Exam Form” that is provided as a downloadable form)
• NYSED Interval Health History Form (electronic signature required)
• Interscholastic Rules and Regulations (electronic signature required)
• Concussion Guidelines/Sudden Cardiac Arrest in Youth (electronic signature required)
• Electronic Signature Agreement (electronic signature required)

If you are a high school student who is transferring into the district, we require you to complete this form. Transfer Registration Form for Athletic Department Please email the completed form to bwilson@mw.k12.ny.us 

Physicals for Freshman, JV & Varsity athletes must be dated March 1, 2024 or later.

Physicals for Modified athletes must be dated March 1, 2024 or later.
